About Us

We operate from 2 practices.
The main site being:
Pallion Health Centre, Hylton Road.
Sunderland. SR4 7XF.
Monday 8.30am – 7.30pm
Tuesday – Friday 8.30am – 6.00pm

The branch site:
Ryhope Health Centre, Black Road. Ryhope.
Sunderland. SR2 0RY

Monday Tuesday Wednesday and Friday 8.30am – 5.30pm
Thursdays 7.30am start

Areas covered by the practice

We register patients living in the following areas:
Sunderland SR1-SR6

Registration is easy – please complete the GMS1 form and bring to the surgery (Pallion or Ryhope) with proof of ID.

As part of the registration process, you will be asked to complete a Health Questionnaire and Patient / Practice Contract when you next visit the surgery.

Please be assured that when registering new patients Hylton Medical Group does not discriminate on the grounds of gender, creed, sexual orientation, age, colour, race, appearance, disability or medical condition.

Should you have any queries about the registration process then please do not hesitate to contact us or call in and speak to one of our helpful receptionists.

We have made the form as simple as possible for your ease of completion.

Submitting this form does not constitute or imply automatic registration or acceptance onto the register of patients at Hylton Medical Group.

Once you have submitted this form, please attend the surgery in the next 5 working days BRINGING WITH YOU:

  • Any medical cards or NHS numbers for each person registering.
  • The Yellow Book for any children aged under 16.

Proof of address AND identity (please note we will only accept the following):

  • Valid driving licence
  • Valid passport plus proof of address
  • Current local authority rent card
  • A utility bill (dated no more than three months ago)
  • Recent bank or building society statements
  • Recent payslip plus proof of address or P45
  • Recent documentation from DWP showing name and address

To complete your registration you will be asked to fill out a medical questionnaire and sign a practice / patient contract (and your registration form if completed online).  Thank you for choosing Hylton Medical Group.

WE WILL NOT BE ABLE TO REGISTER YOU IF YOU ARE UNABLE TO PROVIDE PROOF OF ADDRESS AND IDENTITY.

As from 1st April 2015 the practice is required to allocated all patients with a named accountable GP, who has overall responsibility for their care.  To find out who you named GP is please contact the practice on 01915229908 and the admin staff will be happy to inform you.

To register for online services with the practice please contact the practice and ensure you have 2 forms of ID as specified below:

Proof of address AND identity (please note we will only accept the following):

  • Valid driving licence
  • Valid passport plus proof of address
  • Current local authority rent card
  • Other form of photographic ID
  • A utility bill (dated no more than three months ago)
  • Recent bank or building society statements
  • Recent payslip plus proof of address or P45
  • Recent documentation from DWP showing name and address

Once we have verified your ID you can be issued with the appropriate access ID; then follow the following link:

The online services lets you book and cancel your appointments and also allows you to request your repeat medication and view your allergies.  From April 2016 You will also be able to have access to your medical summary.  Due to the extra access we are under strict instructions regarding rigorous ID verification.
